As an additional service to my clients, I can have your last name changed back to a maiden or previous surname. You can download a simple form here and include it with the divorce forms to start your divorce along with the additional fee of $75.00. I will then prepare all the documents required and file them with the court. I do all the work. After the court records the name change, they will issue a document that will be sent to you. With that document, you will be able to change the name on your social security card, drivers license and other important papers. |
